Left Shift or Lip Service? Why the NHS can’t afford to get this wrong

“Left shift” has become one of the NHS’s most repeated ambitions: moving care upstream, closer to home, more preventative, more personalised, and less hospital-centric. But despite years of policy intent, pilot programmes and rhetoric, acute demand continues to rise, health inequalities widen, and staff feel stretched thinner than ever.

This panel asked the uncomfortable question: Is left shift genuinely achievable at scale, or has it become a convenient narrative that masks under-investment, risk transfer and system inertia? Drawing on frontline experience, system leadership, workforce realities and community perspectives, the discussion will cut through slogans to examine what it would actually take to make left shift real – and what will happen if we don’t.

As a group they discussed how collaboration, money, benefits, accountability, and culture all have a part to play on how the NHS can be structured to deliver care earlier and closer to people’s everyday lives.
